Welcome to the Enrollment Center! Here you will find all the necessary information and tools to ENROLL or WAIVE COVERAGE in the Agnes Scott College Student Health Plan.

The 2022–2023 Agnes Scott College Student Health Plan, underwritten by Sirius America Insurance Company, is for all full-time students and their eligible dependents. You are required to have health insurance to attend Agnes Scott College, both to protect against unexpected high medical costs and provide access to quality care. Please review the brochure for details related to the benefits, limitations, exclusions and costs before starting the enrollment process.

OPEN ENROLLMENT:
Open Enrollment for all eligible dependents ends June 22, 2022. If you have eligible dependents and you choose not to enroll your dependents for coverage before the Annual Enrollment Deadline of June 22, 2022, your dependents will not be eligible to enroll again until the start of next Fall unless you experience a “Qualifying Life Event” during the year.

Login to the site using the login credentials. (Username = Agnes Scott College Email Address Password = DOB in format MMDDYYYY). The site will walk you through the enrollment process

You will be automatically enrolled in this plan unless you WAIVE the coverage by providing proof of comparable coverage prior to the Waiver Deadline. Costs of the plan are collected through Agnes Scott College's tuition billing system. The Agnes Scott College Student Health Plan can only be waived if you have comprehensive medical coverage under another insurance plan that meets our requirements.

WAIVER DEADLINES:
Fall Waiver Deadline (New Incoming Students Only): June 22, 2022
If you do not submit an approved waiver before the deadline, you will be automatically charged and enrolled in the Agnes Scott College Student Health Plan. Once enrolled, there are no refunds or cancellations. (Please note: automatic enrollment will occur AFTER the waiver deadline.) If you choose to WAIVE the insurance coverage, you will not be eligible to enroll until the following school year unless you experience a Qualifying Life Event.

If you determine that you may WAIVE the insurance coverage and have other insurance, you may need to provide the following information:
• Insurance Company Name
• Policy Number
• Group Number (if applicable)
• Policy Holder Name
• Insurance Company Phone
• Insurance Company Address

You will also need to answer specific questions related to your comparable coverage that will determine if you can WAIVE the insurance coverage.

Once you have successfully completed the waiver steps through this site, an email will automatically be sent to the email address you provided notifying you of the waiver request determination. If you receive a:
Approval email– Your waiver was approved. You should print this e-mail and save it with your records.
Denial email– Your waiver was denied. If you would like us to look this over or if you answered a question incorrectly, please contact us.

If you have missed the waiver deadline of June 22, 2022, but have not previously waived out of the insurance, you will be automatically enrolled in this plan and be billed the coverage costs through your tuition billing.

If you have previously waived out of the insurance but have since lost coverage due to a Qualifying Life Event, you will need to complete a Qualifying Life Event Change Form. Please contact us to receive a Qualifying Life Event Change Form and instructions to submit payment for the insurance coverage.

Qualifying Life Event
If you did not enroll yourself or any eligible dependents during open enrollment and have since lost coverage under your original comparable plan, you and/or your eligible dependents may qualify for a Qualifying Life Event upon providing proof of involuntary loss of coverage and payment within 30 days of losing coverage. The Qualifying Life Event Change Form, check or money order, and the letter of creditable coverage must be received within 30 days of losing coverage. If you do not submit the completed Qualifying Life Event Change Form and the required costs of the coverage, you will no longer be eligible to enroll in the plan until Fall of the following school year.

Qualifying Life Events include:
• Acquiring a new family member
• Marriage/Civil Union
• Divorce
• Loss or change of Job
• Expiration of Benefits

If you and/or your dependents meet the requirements for a Qualifying Life Event, you will need to provide the following:
• Proof of qualifying life event/proof of involuntary loss of coverage
• Qualifying Life Event Change Form
• Payment
